What do you know about SinglePoint end of life care services in North Essex & Colne Valley?
SinglePoint is a palliative and end of life care service which works alongside other services you receive from your GP, community nurses or specialists, to make sure you get high quality coordinated care at home. The SinglePoint service is run from our Highwoods, Colchester site.
SinglePoint was developed following a review of palliative and end of life services in the locality. A number of organisations – St Helena Hospice, Colchester Hospital University NHS Foundation Trust, Anglian Community Enterprise (ACE), East of England Ambulance Service and North East Essex Clinical Commissioning Group – are working collaboratively providing this integrated service. This service is for those in North East Essex and the Colne Valley area of Mid Essex.
